 The Contact information may be considered protected health information. Your consent authorizes that this information can be released in written and verbal form. The MHPSOGFW will not be responsible for any data or messaging charges. 

 Please understand that your participation with the Mental Health Peer Services of Greater Fort Worth will not be affected if you do not allow consent. One major advantage to providing consent is you will be alerted to group changes or other organization information.
